Script Digital Forms:
The following forms can be completed and submitted online using Script:

  • Compensatory Pay Timesheet

  • Conference Request

  • Expense Report

  • IEP Writing Timesheet

  • Mileage Report

  • Professional Learning Planning Timesheet

  • Tuition Reimbursement Request

Instructions on how to complete & submit:
Go to schools.scriptapp.com

  • Enter your BASD email address

  • Click NEXT

  • Click the option to "Sign in with Google" and follow the prompts

  • Click School Links to access the following forms

  • Click Start New Submission to open a form or timesheet

  • Complete Form

  • Click Submit in the upper right hand corner and once completed you should receive a confirmation E-Mail of your submission

Payroll Forms:

  • Direct Deposit

  • Current Year W-4 

  • 403B Deduction Information

  • Mileage Chart

  • Mileage Report

  • Expense Report

Timesheets:
