Volunteers

Athletic Volunteers must complete the below paperwork.  Once all paperwork is complete, please turn into the Athletic Director.  At that time, your name will be forwarded to be put on the agenda to be Board approved.  This must be done at least two weeks prior to the next board meeting.

  1. Volunteer Form,

  2. Statement of Confidentiality, (2nd page of volunteer form)

  3. Residency Affidavit & Arrest/Conviction Report,  (if you have not lived in PA for the last 10 years you will also need to submit an FBI Fingerprint Report*)

  4. PA Child Abuse History Certification, (must be dated within the last 5 years)

  5. PA Criminal History clearance,  (must be dated within the last 5 years)

  6. Mandated Child Abuse Report Training (must be completed every 5 years)

  7. Suicide Prevention Course (must be completed every 5 years)

  8. Results of a TB test (one time only)

  9. Concussion (must be completed every year dated after June 1st)

  10. Sudden Cardiac Arrest (must be completed every year dated after June 1st)

  11. PIAA Coaching Courses - Fundamentals of Coaching and First Aid.  Use this link and follow directions.  

 *Note: If you have not lived in PA during the past 10 years, you will also need an FBI Fingerprint Report. Click here if you need to complete this clearance.  Enter service code 1KG6Y3, if you are only using this for volunteer purposes (if you also need this for employment, please speak to your employer regarding the appropriate service code).  Any report that has been run since you have moved to the State is acceptable.   ONLY THOSE WHO HAVE NOT LIVED IN PA IN THE PAST 10 YEARS NEED TO GET AN FBI REPORT - THIS IS NOT REQUIRED OF ALL VOLUNTEERS. THERE IS A $28 FEE FOR THIS SERVICE, PLEASE DO NOT DO THIS IF YOU DON'T NEED IT.
